[JSR308] Locations for annotations on types

The JSR 308 proposal lists the locations that annotations must be permitted
in order to allow annotating any use of a type.  This list appears at the
beginning of section 3 (for source code) and in figure 2 (for class files).

Please let me know if either list is missing any locations.  Thanks!
